No. Small, partial tears in older, low-demand patients often respond well to targeted physiotherapy, activity modification, medication and, where indicated, a corticosteroid injection. Surgery is offered for larger or full-thickness tears, tears in active or younger patients, and tears that have not responded to conservative treatment.
Most modern rotator cuff repairs are performed arthroscopically through small keyhole incisions using a camera and specialised instruments. This causes less soft-tissue damage and generally faster early recovery. Very large or complex tears may occasionally require a mini-open approach.
Tendons that have torn away from bone do not typically heal back to bone on their own but symptoms from many partial tears can improve significantly with focused rehabilitation.
