When Does an ACL / PCL Tear Need Surgery?

ACL and PCL reconstruction is performed arthroscopically using either your own tendon (autograft hamstring, patellar or quadriceps) or, in selected cases, a donor tendon (allograft). The graft replaces the torn ligament and, over months, remodels into a functional new ligament. Rehabilitation is structured and progressive: initial protection and range of motion in weeks 1–6, strengthening in weeks 6–16, sport-specific training in months 4–6, and return to competitive pivoting sport around 9 months, based on your surgeon’s clearance.

Some patients usually older, low-demand individuals not participating in pivoting sports can manage a torn ACL with focused rehabilitation and activity modification. However, active patients, athletes and those with recurrent instability generally need reconstruction to safely return to sport and to protect the meniscus and cartilage from further damage.
The most common graft choices are hamstring tendon, patellar tendon (bone-patellar-tendon-bone), quadriceps tendon and, in selected cases, allograft (donor tendon). The choice depends on your age, activity level, sport, previous surgery and your surgeon's assessment.
Return to competitive pivoting sport (cricket bowling and fielding, football, kabaddi, basketball) typically happens around 9 months after ACL reconstruction, and only after your surgeon confirms adequate strength, stability and sport-specific readiness. Return too early significantly increases the risk of re-injury.
